The US Chemical Safety and Hazard Investigation Board (CSB) is investigating an explosion that killed 16 workers at an Accurate Energetic Systems (AES) facility in Tennessee.
The incident occurred on Oct. 10 at the AES facility in Humphreys County, about 80 km west of Nashville, where the company manufactures explosive products for defense and commercial markets.
"This is one of the deadliest industrial incidents in our country in years,"
says CSB chairperson Steve Owens in a statement.
