Primary Health Network (PHN) has officially reopened the Shenandoah Health Center at 624 W.
The facility, which had been temporarily inactive, will once again provide accessible family medicine services, including primary care and minor sick visits, as of October 6th, 2025.
The center will be led by Yalixsa Then, CRNP, a Certified Registered Nurse Practitioner specializing in family medicine, who joined PHN in September.
Then brings extensive experience and a strong educational background, having earned her bachelor’s degree in nursing from Long Island University in Brooklyn, NY, and her Master of Science in Nursing from Lehman College in the Bronx, NY.
PHN announced plans to expand its Shenandoah team in the future, further improving access to community-based care and ensuring long-term sustainability.
As a Federally Qualified Health Center (FQHC), PHN provides high-quality care to all patients, regardless of income, insurance status, or background.
